Case Manager/Youth Advocate, La Cultura Cura

Agency Description: Instituto Familiar de la Raza, Inc. is a multi-service community health and social service agency with emphasis in serving the Chicano/Latino community in San Francisco with a special focus on the diverse needs of the Mission District. Services include a wide range of mental health, HIV related services, and social services including advocacy, early intervention, psychotherapy, psychiatric care, and case management for adults, youths, children and families.

Program Description: Instituto Familiar de la Raza/La Cultura Cura offers a range of services and opportunities to youth and their families in the juvenile justice system. Youth and families are provided with case management and mental health services in the community and have access to culturally oriented structured activities. The Case Manager will be located three days at CARC’s facilities and two days at Instituto’s.

Essential Duties: Duties include, but are not limited to the following:
Provide psychosocial assessment, comprehensive plans of care, follow-up and monitoring of chart plan of program participants
Attend juvenile court proceedings and make curfew calls as needed
Conduct school, home and community visits.
Plan and implement educational and recreational activities for youth.
Participates in collaborative meetings, inter-agency trainings and meetings, and other events as requested.
Document all encounters/contacts in the client records and maintain statistical data as requested.
Complete monthly reports to Project Coordinator and to funding sources when needed
Assist minors’ Probation officer by reporting non-compliant minors, on-going problems with minors and increasing interventions when needed
Other duties as assigned

Required Skills:
B.A. in social sciences or a comparable combination of education and training in the areas of social welfare, youth development and/or community advocacy
At least two years of experience providing services to youth in and out the juvenile justice system
Knowledge of case management practices
Bi-lingual English-Spanish
Knowledge of and sensitivity to issues of diversity in the Latino/Chicano community, especially around sexual orientation, culture, language and gender

Preferred Skills:
Demonstrated competency in working with the Chicano/Latino community
Excellent organizational and time management skills
Ability to work as member of a multi-disciplinary team
Experience in a non-profit community agency

Compensation & Benefits: Salary Range: $32,000-$36,000. Benefits include medical, dental, retirement plan, access to vision plan and chiropractic & acupuncture insurance, thirty-five hours work-week, paid lunch, paid vacation, sick leave and holidays.
